HOW TO ORDER YOUR TEST KIT
Your COVID-19 Shallow Nasal RT-PCR Test Kit(s) will be delivered after you complete the health screening survey and create an account to receive a lab order on the myLAB Box website. After checkout, you will receive a coupon at the email address you provided from email@example.com for each test kit that you purchase on samsclub.com. Coupons are normally delivered within an hour, though more processing time may be required. The email will include instructions for how to redeem your coupon code, complete a health screening survey, and create an account for a lab order to receive your test kit(s). You must complete these steps BEFORE a kit will be shipped to you.
Your test kit(s) will be delivered after you complete the health screening survey and account set up to receive a lab order on the myLAB Box website. Your test kit(s) will be shipped the next day for all accounts completed by 11:00 AM CST Monday - Thursday (AK and HI are second-day shipping where Next Day is not available). Accounts completed after 11:00 AM CST on Friday or Saturday will deliver on Monday. Ac-counts completed after 11:00 AM CST on Sunday will deliver Tuesday.
A health screening survey and account set up must be completed for each person testing to receive a lab order and test kit. Each adult must use their own email address, create a unique account, complete the health screening survey and set up an account. If you are ordering a test for a dependent, you will create your account and add your dependent as the patient.
IMPORTANT NOTICE: If you have any of the following emergency warning signs and symptoms of a serious COVID-19 infection (difficulty breathing or shortness of breath, persistent pain or pressure in the chest, new confusion or inability to arouse, bluish lips or face), please do NOT order a test for COVID-19. Please instead call your health care provider immediately, and failing to reach them, go to the nearest emergency room or dial 911.
You must supply an email address at check out to receive your coupon code. Please add firstname.lastname@example.org to your Email Address Book or Safe List to ensure delivery of your code.
Coupon codes that have not been redeemed are only eligible for return within 30 days of purchase. Once a kit has been shipped, it is not eligible for return. Members must call Sam's Club Customer Service to process the return of the coupon code. Return processing of coupon codes is unavailable through warehouse locations.
